C&C PHARMACY COVID-19 VACCINE UPDATE

BACKGROUND INFORMATION:

C&C Pharmacy currently has the Johnson & Johnson COVID-19 Vaccine. Vaccine supply is limited and will be provided on a first come, first serve basis. To schedule an appointment, which is required by the CDC, the process listed below must be followed.

The state of Kentucky is currently administering COVID-19 vaccines to people that fall in the following PHASES. Phase 1A, 1B and 1C. Click here to see a detailed breakdown of each Phase to see if you qualify.

PROCESS:

1. Review the PHASE document by following the link above to ensure you qualify.
2. Click here to review the Johnson & Johnson vaccine EUA (Emergency Use Authorization) document.
3. Click here to review and enroll in, if desired but is not required, to the V-safe Program.
4. Complete and submit the on-line questionnaire by clicking here.
   a. There is no need to sign the document. We will print it at the pharmacy, and you will sign the day you receive the vaccination.
5. The on-line questionnaire will be sent directly to C&C Pharmacy for review.
6. You will receive a call form a C&C Pharmacy Staff member to schedule your vaccination.
